Form 21-4138 - Statement In Support Of Claim

The Statement In Support Of Claim form is where you detail what is wrong with you and how it affects your daily life. If there is more than one thing wrong with you, list each one individually.

Be clear about what you are applying for. If you have several health problems, list each of them and their impact on your daily activities. Work with your representative to be specific about the disability(s) you list. This is a very important document and tells the VA why you're filing the claim.

You can always give the VA additional Statements later on. Doing so will probably slow down the processing of your claim but if you feel it is important enough, by all means give the VA as much information as possible.

The best place to get this form is either from the VA or your Veteran Service Representative. The VA will send you a couple of copies when they send the packet of information after you start your claim.
