Steps To Filing A Claim With The VA for
PTSD, Agent Orange Related Illnesses or Other Disability

1) Talk to a Veteran Service Representative telling them you want to open a claim. More Info

2) Obtain a copy of your Report of Separation, also known as a DD-214, and/or your Official Military Personnel File (OMPF) from the military. More Info

3) Complete VA Form 21-4138, Statement In Support Of Claim. More Info

4) File the claim with the local Department of Veterans Affairs (VA).

5) Wait to hear from the VA. They will write to you telling you they have received your claim and which team at the VA will be handling your claim and how to reach them. More Info

6) In the meantime, continue to gather information that will support your claim. This includes documents such as Combat After Action Reports, Morning Reports and Daily Staff Journals, Clinical Records which show you being treated in Vietnam for wounds, illnesses and other medical treatment. These are all obtainable from various government sources. More Info
